1. Fully Filled application form to be submitted
2. At the time of submission of form, attach the following
a. One recent passport size photograph pasted on the form duly signed by the applicant.
b. One stamp size photograph.
c. Original pre-B.Ed. Mark sheet + 10 photocopies (for B.Ed. student)
d. Photocopy of 10th,12th Graduation/post graduation mark-sheet (whichever is applicable)
e. Photocopy of domicile certificate.
f. Photocopy of caste certificate. (for SC/ST/OBC student)
g. Photocopy of class certificate. (If applicable)

3. At the time of confirmation of admission, submit the following:
a. 5 passport size photographs +2 stamp size photograph
b. Five attested photocopies each of 10th, 12th Graduation, past Graduation mark sheets 
c. Five attested photocopies of caste certificate 
d. Five attested photocopies of caste certificate [ For SC/ST/OBC Students] 
e. Five attested photocopies of class certificate. [If applicable]
f. Original School/College transfer certificate +5 photocopies.
g. Original Gap certificate/Affidavit [If applicable] +five photocopies.
h. Original Name change certificate (If applicable) + five photocopies
i. Income Certificate (if applicable) + five photocopies
j. Original Migration certificate (If applicable) + five photocopies
k. Original Eligibility Certificate from Jiwaji University+ five photocopies (if applicable)
l. Photocopy of enrolment No. of Last Exam (if passed from Jiwaji University)

4. Admission will be confirmed only after the submission of all the above said documents and full fees payment.
